Summary: Background. Little is known about the relationship between allergic diseases and seizure disorders including epilepsy. It is hypothesized that inflammation from allergic diseases may predispose children to seizures. the aim of this study is to investigate frequency of seizure disorder in children with asthma and allergy. Methods. A cross-sectional survey study of parents of 1300 children and adolescents under 20 years of age referred to the Allergy and Asthma Clinic of Imam Ali Hospital (Karaj) who were asked to complete a screening questionnaire for seizures in their children. Parents who reported any history of seizures in their children were contacted to answer a second in-depth questionnaire to determine more detail of type, triggers, and treatment of seizures. Results. A total of 705 males (62%) and 433 females (38%) participated in this study, with a mean and standard age of 6.62±4.57 years. Among them, 70.6% had asthma, 15.2% had allergic rhinitis, 5.6% had atopic dermatitis, 3.5% had urticaria, 2.7% had food allergies, 1% had drug allergies, and 1.4% had other allergic diseases. Additionally, 88 patients (7.7%) had a history of doctor-diagnosed seizures, 57 patients (5%) had febrile convulsions, and 15 patients (1.31%) had idiopathic epilepsy. There was no significant relationship found between febrile convulsions, seizures, and epilepsy with the type of allergic diseases. However, a significant association was observed between the number of comorbid allergic diseases in patients with febrile convulsions (OR=1.4, 95% CI: 1.07-1.83, P=0.013).There was also an association between the epilepsy and comorbid allergic diseases number with an odds ratio OR=1.84, 95% CI=0.28-12, however the risk of epilepsy was increased by 0.84 percent but this increase was not significant. Regarding the relation between the number of allergic diseases in parents and idiopathic epilepsy in their children, a significant association was found only for mothers (OR=1.28, 95% CI: 1.04-2.23, P=0.024), but not for fathers (P>0.05). Conclusions. Febrile convulsion is associated with the .number of comorbid allergic diseases in children and the mother's number of allergic diseases is more related to idiopathic epilepsy in children than the father's.

BACKGROUND: Citral (C10H16O) is the main ingredient of Cymbopogon citratus (lemongrass oil) and can reduce the side effects of oxidative stress. Diabetes caused by insulin deficiency induces oxidative stress in the liver. AIMS: This study aimed to investigate the ameliorative effects of citral on selected oxidative parameters and the gene expression of paraoxonase 1 (PON1) and endothelial nitric oxide synthase (eNOS) in a rat model of streptozotocin (STZ)-induced diabetes mellitus. METHODS: Forty rats were divided into four groups at random: control (C), control citral (CC), and two STZ-induced diabetic groups (diabetic (D) and citral diabetic (CD)). After diabetes confirmation (day 7), gavage treatment with citral (300 mg/kg body weight (BW)) was started in the CD and CC groups and continued for two weeks. RESULTS: On day 21 of the study, following treatment with citral for 14 days, the serum levels of total antioxidant capacity (TAC), and PON1 in the CD group were significantly increased compared to those in the D group (P<0.05). While treatment with citral caused a significant decrease in the Malondialdehyde (MDA), and eNOS in the CD group compared to those of the D group (P<0.001). The expression rate of liver PON1 gene was considerably upregulated in the CD group compared to that in the D group (P<0.001); while the opposite was observed for eNOS gene expression. However, there was no significant difference between the CC and C groups in terms of all examined parameters (P>0.05). CONCLUSION: This study showed positive effects of citral on serum antioxidant status and liver gene expression of PON1 and eNOS in diabetic rats.

Diabetes mellitus is a global endemic with a rapidly increasing prevalence in both developing and developed countries. Recently, hemoglobin A1c has been recommended by the American Diabetes Associations as a possible substitute for fasting blood glucose for the diagnosis of diabetes, because it is an indicator of long-term glycemic control. Also, centrifugal microfluidic systems have good potential for use in the point of care testing systems. In this study, a centrifugal microfluidic disc was designed and manufactured to measure hemoglobin A1c in whole blood using an immunoturbidimetry based method. Also, a new passive valve, named septum valve, was presented to precisely control the entry and exit of reagents. This design comprises three inputs for injection of reagents and a blood sample, three septum valves, a two-part mixing chamber and a chamber to measure the absorbance of the sample based on the immunoturbidimetry method. Fourteen blood samples were tested using the manufactured disc, and the results were very congruous with the clinical data. By using the designed centrifugal microfluidic disc, the hemoglobin A1c in whole blood was measured in 8 min, with a standard deviation of ±â€¯0.36% HbA1c.

We consider a hybrid quantum many-body system formed by a vibrational mode of a nanomembrane, which interacts optomechanically with light in a cavity, and an ultracold atom gas in the optical lattice of the out-coupled light. The adiabatic elimination of the light field yields an effective Hamiltonian which reveals a competition between the force localizing the atoms and the membrane displacement. At a critical atom-membrane interaction, we find a nonequilibrium quantum phase transition from a localized symmetric state of the atom cloud to a shifted symmetry-broken state, the energy of the lowest collective excitation vanishes, and a strong atom-membrane entanglement arises. The effect occurs when the atoms and the membrane are nonresonantly coupled.

Y chromosome has a number of genes that are expressed in testis and have a role in spermatogenesis. TTY2L12A and TTY2L2A are the members of testis transcript Y2 (TTY2) that are Y linked multi-copy gene families, located on Yp11 and Yq11 loci respectively. The aim of this study was to investigate frequency of TTY2L12A and TTY2L2A deletions in azoospermic patients compared with fertile males. This study was performed on 45 infertile males with idiopathic azoospermia without any AZF micro deletions (group A), 33 infertile males with azoospermia which do not screened for AZF micro deletions (group B) and 65 fertile males (group C), from October 2013 to April 2015 in west of Iran. Polymerase chain reaction (PCR) method was used for detection of TTY2L12A and TTY2L2A gene deletions in studied groups. No deletions were detected in normal fertile males of group C. 1 out of 45 azoospermic males of group A (2.22%) and 3 out of 33 azoospermic males of group B (9.09%) had TTY2L2A deletion (p= 0.409 and p= 0.036 respectively), also 1 out of 45 azoospermic males of group A (2.22%) and 4 out of 33 azoospermic males of group B (12.12%) had TTY2L12A deletion (p= 0.409 and p= 0.011 respectively).  None of azoospermic males in Group A and B had deletions in both genes. Our data showed significant correlation between non-obstructive azoospermia and TTY2L12A and TTY2L2A deletions. Thus, it seems that TTY2L12A and TTY2L2A deletions can consider as one of the genetic risk factors for non-obstructive azoospermia.

INTRODUCTION & PURPOSE: Perinatal loss is one of the toughest events of life. Physiological milk secretion after perinatal loss adds to complicacy of the hardships of the event. The present study is aimed at exploring women's experience with breast problems and milk leakage after perinatal loss. METHODS: The Study was carried out through explorative quality approach with 18 participants. Sampling method was purposeful and selecting the participants from widest variety was ensured. Data gathering was through deep semi-structured interview and data analyses were done by conventional content analysis. Reliability and validity of the data were ensured by collecting data from a wide range of participants and frequent revisions. FINDINGS: Data analysis indicated four themes including beyond pain, longing being mother, insufficiency of provided information and coping Strategies, and beliefs and values regarding milk leakage and breast engorgement. CONCLUSION: The findings suggested that health care givers needed to inform the patients about probability milk leakage and breast engorgement and remedies to reduce pains and problems of breast engorgement.

RESUMO

It is well known that the bosonic Hubbard model possesses a Mott insulator phase. Likewise, it is known that the Dicke model exhibits a self-organized superradiant phase. By implementing an optical lattice inside of a high-finesse optical cavity, both models are merged such that an extended Hubbard model with cavity-mediated infinite range interactions arises. In addition to a normal superfluid phase, two superradiant phases are found, one of them coherent and hence superfluid and one incoherent Mott insulating.

We investigate the nonlinear light-matter interaction of a Bose-Einstein condensate trapped in an external periodic potential inside an optical cavity which is weakly coupled to vacuum radiation modes and driven by a transverse pump field. Based on a generalized Bose-Hubbard model which incorporates a single cavity mode, we include the collective backaction of the atoms on the cavity light field and determine the nonequilibrium quantum phases within the nonperturbative bosonic dynamical mean-field theory. With the system parameters adapted to recent experiments, we find a quantum phase transition from a normal phase to a self-organized superfluid phase, which is related to the Hepp-Lieb-Dicke superradiance phase transition. For even stronger pumping, a self-organized Mott insulator phase arises.

Introduction: Osteoporosis has increased the burden of diseases worldwide and in all populations. It is far more common among women than among men. The current research tried to investigate the health faculty students' awareness of osteoporosis (calcium intake and physical activity). Materials & methods: A descriptive-analytical research was conducted on 239 students in the health faculty, who voluntarily participated in this study and chose to use the census instancing approach. The information gathered employed a standardized questionnaire including 9 demographic questions, and 24 questions about osteoporosis were fed into SPSS 18 statistical software, t-test, Pearson's correlation, and one-way ANOVA. Findings: 228 students (95.4%) enrolled at the undergraduate level. They age averaged 22.17 ± 2.66. Their mean score of knowledge regarding osteoporosis was 12.96 ± 4.01 (4.67 ± 1.66 for calcium intake and 8.29 ± 2.89 for physical activity section). Only 46 (19.2%) students had a high-level knowledge of osteoporosis. There was a clear link (P < 0.05) among the students' awareness regarding the marital status, being older, being a BA student of public health, and being a MA. There was no clear link among the awareness and the students' job, parents' job, education, and family income (P > 0.05). Discussion & conclusion: Given the students' field of study, that being health sciences, their awareness about osteoporosis was poor and unacceptable. Therefore, it was essential to promote the students' knowledge and determine the causes of insufficient physical activity and calcium intake among students.

Diabetes leads to reproductive dysfunctions by producing free radicals. It seems that using walnut can be effective in the damage induced by diabetes. The aim of the present study was to evaluate the effects of walnut consumption on sex hormones in diabetic Wistar male rats induced by STZ (streptozotocin). Diabetes animals were induced by STZ (60 mg/kg). Rats were randomly divided into 5 groups (n=6), including normal diet and healthy (Sham), Diabetic by normal diet (control) and diabetic by 6, 9 and 12% walnut in their diet (experimental groups), and were examined for 6 weeks. Body weight, blood glucose (3 times), and sex hormones (testosterone, FSH and LH) were measured. Overall, in terms of the mean index, there was a significant difference in the percentage of weight changes between the groups (p<0.001). Blood glucose (3 times) significantly increased in experimental and control groups in comparison with sham group (p<0.001). FSH concentration significantly decreased in control group (p<0.05) and testosterone hormone decreased in experimental and control groups compared to sham group (p<0.05). Oral administration of walnut seems to prevent severe weight loss in the experimental models of diabetic rats and exerts appropriate and useful changes in blood glucose level as well as positive effects on the secretion of sex hormones.


La diabetes conduce a disfunciones reproductivas mediante la producción de radicales libres. Parece que el uso del nogal puede ser eficaz para contrarestar el daño inducido por la diabetes. El objetivo fue evaluar los efectos del consumo de nueces sobre las hormonas sexuales en ratas Wistar macho diabéticas, inducidas por estreptozotocina (STZ). La diabetes en los animales fue inducida por STZ (60 mg/kg). Los animales fueron divididos aleatoriamente en 5 grupos (n= 6 ): saludable con dieta normal (Sham), diabéticos con dieta normal (control) y diabéticos con consumo de nogal en 6, 9 y 12% en su dieta (grupos experimentales), quienes se examinaron durante 6 semanas, donde se midieron el peso corporal, glucosa en la sangre (3 veces) y hormonas sexuales (testosterona , FSH y LH). En general, en términos del índice promedio, hubo una diferencia significativa en el porcentaje de cambios de peso entre los grupos (p<0,001). La glucosa en sangre aumentó significativamente en los grupos experimentales y de control en comparación con el grupo Sham (p<0,001). La concentración de FSH se redujo significativamente en el grupo control (p<0,05); la testosterona disminuyó en los grupos experimentales y control en comparación con el grupo sham (p<0,05). La administración oral de nogal parece prevenir la pérdida severa de peso en los modelos experimentales de ratas diabéticas y ejerce cambios apropiados y útiles en el nivel de glucosa en la sangre, así como efectos positivos sobre la secreción de hormonas sexuales.

We theoretically investigate the thermodynamics of an interacting inhomogeneous two-component Fermi gas in an optical lattice. Motivated by a recent experiment by L. Hackermüller et al., Science 327, 1621 (2010), we study the effect of the interplay between thermodynamics and strong correlations on the size of the fermionic cloud. We use dynamical mean-field theory to compute the cloud size, which in the experiment shows an anomalous expansion behavior upon increasing attractive interaction. We confirm this qualitative effect but, assuming adiabaticity, we find quantitative agreement only for weak interactions. For strong interactions we observe significant nonequilibrium effects which we attribute to a dynamical arrest of the particles due to increasing correlations.

OBJECTIVES: Skin-derived precursors are recognized to be a potentially autologous and accessible source of neural precursor cells for drug screening or cell-based treatments, in many neurological disorders. Thus, it is necessary to investigate appropriate methods for cryopreservation of such human skin-derived precursors (hSKPs). The aim of this study was to evaluate different cryopreservation techniques for retention of hSKPs to discover an optimized protocol. MATERIALS AND METHODS: We cryopreserved hSKPs treated with 0%, 10%, 20%, 30% and 40% foetal bovine serum (FBS) and three concentrations of dimethylsulphoxide (DMSO) 5%, 10% and 15%, with two different storage periods in liquid nitrogen (2 days: short-term storage; and 2 months: long-term storage). Then, we assessed survival and proliferation levels of the cells after freeze-thaw processes, by viability measurement and colony-forming assay. For detecting hSKPs, we used immunocytochemistry and RT-PCR assessments. RESULTS: Our findings indicated that hSKPs cryopreserved in 5% DMSO without FBS, had better survival and proliferation potentials compared to other working formulations. With various concentrations of cryoprotectants over different time periods, hSKPs retained their differentiation potentiality and were able to differentiate into neurons (NFM and ßΙΙΙ tubulin-positive), glial cells (GFAP-positive) and smooth muscle cells (SMA-positive). CONCLUSIONS: Results revealed that in only 5% DMSO, hSKPs could be cryopreserved for long-term storage with considerable survival and proliferation levels, without losing multipotency.

PURPOSE: Since 2010, there has been a great attention about HTML5. Application developers and browser makers fully embrace and support the web of the future. Consumers have started to embrace HTML5, especially as more users understand the benefits and potential that HTML5 can mean for the future.Modern browsers such as Firefox, Google Chrome, and Safari are offering better and more robust support for HTML5, CSS3, and JavaScript. The idea is to introduce the HTML5 to medical physics community for open source software developments. The benefit of using HTML5 is developing portable software systems. METHODS: The HTML5, CSS, and JavaScript programming languages were used to develop several applications for Quality Assurance in radiation therapy. The canvas element of HTML5 was used for handling and displaying the images, and JavaScript was used to manipulate the data. Sample application were developed to: 1. analyze the flatness and symmetry of the radiotherapy fields in a web browser, 2.analyze the Dynalog files from Varian machines, 3. visualize the animated Dynamic MLC files, 4. Simulation via Monte Carlo, and 5. interactive image manipulation. RESULTS: The programs showed great performance and speed in uploading the data and displaying the results. The flatness and symmetry program and Dynalog file analyzer ran in a fraction of second. The reason behind this performance is using JavaScript language which is a lower level programming language in comparison to the most of the scientific programming packages such as Matlab. The second reason is that JavaScript runs locally on client side computers not on the web-servers. CONCLUSIONS: HTML5 and JavaScript can be used to develop useful applications that can be run online or offline on different modern web-browsers. The programming platform can be also one of the modern web-browsers which are mostly open source (such as Firefox).

PURPOSE: A patient-specific quality assurance (QA) method was developed to verify gantry-specific individual multileaf collimator (MLC) apertures (control points) in volumetric modulated arc therapy (VMAT) plans using an electronic portal imaging device (EPID). METHODS: VMAT treatment plans were generated in an Eclipse treatment planning system (TPS). DICOM images from a Varian EPID (aS1000) acquired in continuous acquisition mode were used for pretreatment QA. Each cine image file contains the grayscale image of the MLC aperture related to its specific control point and the corresponding gantry angle information. The TPS MLC file of this RapidArc plan contains the leaf positions for all 177 control points (gantry angles). In-house software was developed that interpolates the measured images based on the gantry angle and overlays them with the MLC pattern for all control points. The 38% isointensity line was used to define the edge of the MLC leaves on the portal images. The software generates graphs and tables that provide analysis for the number of mismatched leaf positions for a chosen distance to agreement at each control point and the frequency in which each particular leaf mismatches for the entire arc. RESULTS: Seven patients plans were analyzed using this method. The leaves with the highest mismatched rate were found to be treatment plan dependent. CONCLUSIONS: This in-house software can be used to automatically verify the MLC leaf positions for all control points of VMAT plans using cine images acquired by an EPID.

We investigate the potential in using of using a graphics processor unit (GPU) for Monte-Carlo (MC)-based radiation dose calculations. The percent depth dose (PDD) of photons in a medium with known absorption and scattering coefficients is computed using a MC simulation running on both a standard CPU and a GPU. We demonstrate that the GPU's capability for massive parallel processing provides a significant acceleration in the MC calculation, and offers a significant advantage for distributed stochastic simulations on a single computer. Harnessing this potential of GPUs will help in the early adoption of MC for routine planning in a clinical environment.

RESUMO

We address an imbalanced two-component atomic Fermi gas restricted by a one-dimensional (1D) optical lattice and an external harmonic potential, within the mean-field Bogoliubov-de Gennes formalism. We show that characteristic features of the Fulde-Ferrell-Larkin-Ovchinnikov state are visible in the rf spectra and in the momentum-resolved photoemission spectra of the gas. Specially, Andreev states or midgap states can be clearly resolved, which gives a direct experimentally observable signature of the oscillating order parameter.

RESUMO

Bremsstrahlung radiation of runaway electrons is found to be an energy limit for runaway electrons in tokamaks. The minimum and maximum energy of runaway electron beams is shown to be limited by collisions and bremsstrahlung radiation, respectively. It is also found that a massive injection of a high-Z gas such as xenon can terminate a disruption-generated runaway current before the runaway electrons hit the walls.

Presence of mutations associated with resistance to zidovudine or lamivudine was determined in isolates of HIV-1 obtained after long-term follow-up of 64 infected individuals who received zidovudine, lamivudine, or both drugs as initial antiretroviral therapy. Zidovudine resistance mutations were less frequent in isolates from patients treated with combination lamivudine plus zidovudine compared with zidovudine alone, but these mutations accumulated over time. Phenotypic resistance to both drugs was found in isolates from 3 of 23 patients. In 3 other patients, lamivudine-resistant virus detected at week 12 was replaced by wild-type virus after longer follow-up, which correlated with a return to baseline levels of plasma HIV-1 RNA. These results show that dual resistance to zidovudine and lamivudine develops over time despite the delayed emergence of zidovudine-resistant mutations. These results also suggest a selective advantage in vivo for HIV-1 species that are wild-type at RT codon 184.

RESUMO

The effect of baseline drug resistance mutations on response to zidovudine, lamivudine, and ritonavir was evaluated in zidovudine-experienced persons infected with human immunodeficiency virus type 1 (HIV-1). Presence of the K70R mutation was associated with significantly higher plasma HIV-1 RNA levels at baseline. However, presence of resistance mutations did not affect the increase in plasma HIV-1 RNA during a 5-week drug washout, nor was there any effect on first-phase virus decay rates after initiation of therapy or on the probability of having plasma HIV-1 RNA levels <100 copies/mL at week 48. Polymorphisms at protease codons 10, 36, and 71 were associated with significantly faster second-phase decay rates. Suppression of plasma HIV-1 RNA despite presence of zidovudine resistance mutations implies that the presence of these mutations does not preclude a durable response to treatment with a potent 3-drug regimen.

OBJECTIVE: To study the effect of HIV-1 resistance to lamivudine (3TC) and zidovudine (ZDV), and syncytium-inducing (SI) phenotype on virologic response to treatment with ZDV, 3TC, or ZDV plus 3TC in previously untreated individuals with HIV-1 infection. DESIGN: A prospective virologic substudy of GlaxoWellcome protocol NUCA 3001. METHODS: HIV-1 isolates obtained at study entry and at week 12 were expanded in peripheral blood mononuclear cell (PBMC) culture, titered, and assayed for phenotypic and genotypic evidence of resistance to ZDV and 3TC, and for syncytium formation on MT-2 cells. RESULTS: Phenotypic and genotypic resistance to 3TC was detected in the majority of HIV-1 isolates from patients who received 3TC alone or in combination with ZDV. Despite showing 3TC resistance, subjects who received 3TC in combination with ZDV had significantly greater decreases in plasma HIV-1 RNA levels compared with those who received ZDV alone. Occurrence of the K7OR ZDV resistance mutation was significantly reduced in patients who received the 3TC/ZDV combination as compared with patients on ZDV monotherapy. Plasma HIV-1 RNA returned to near-baseline levels more quickly in patients with SI isolates at study entry. CONCLUSIONS: Despite the rapid emergence of 3TC resistance, combination therapy with 3TC plus ZDV resulted in greater reduction in plasma HIV-1 RNA levels over 24 weeks as compared to ZDV monotherapy. Prevention of ZDV resistance may contribute to the sustained activity of the combination therapy.
